Create a detailed outline for your disease state presentation. Include 3-5 major bullet points and several supporting points for each requirement.  Your outline should indicate which sources (at least 5) you plan to use within each section. I have included a skeleton outline below that you can use to fill in with details from your specific topic and research.  Also, feel free to use an outline of your own making/choosing.

Following your outline, I would like you to create an annotated bibliography for at least 5 of the sources you plan on using as support in your disease state presentation.  An annotated bibliography is a list of citations to books, articles, and documents.  Each citation is followed by a brief (usually about 150 words) descriptive and evaluative paragraph, the annotation.  The purpose of the annotation is to inform the reader of the relevance, accuracy, and quality of the sources cited.  You are welcome to use citation management software (e.g., EndNote, etc.) to create your annotations or to create them yourselves. 

Two of your sources may come from popular sources (CNN broadcasts, news articles/editorials, popular magazines, etc.), but a minimum of three must come from credible academic sources (found using Google Scholar or other scholarly database search engine available through the library- PubMed, MedLine, SpringerLink, etc.).
