HEALTH ASSESMENT

Select a family to complete a family health assessment.

Before interviewing the family, develop 3 open-ended, family-focused questions for each of the following health patterns:

-Values, Health Perception i.e. individuals’ health status and health practices used to reach the current level of health or wellness

-Nutrition I.e. individuals’ descriptions of food and fluid consumption (history) as well as evidence of adequate nutrition including restrictions, and their perceptions of problems associated with eating and drinking

-Sleep/Rest

-Elimination I.e. Elimination patterns include bowel, bladder, and skin function. Nurses determine regularity, quality, and quantity of stool and urine

-Activity/Exercise

-Cognitive

-Sensory-Perception

-Self-Perception

-Role Relationship

-Sexuality

-Coping

After interviewing the family, compile the data and analyze the responses.
